Nehalem Animal Healing strives to ensure a clear understanding of our financial responsibility with respect to the medical services we provide. These policies apply to all procedures.

Payments: We accept cash, check, debit cards, Visa, MasterCard, and Discover. Nehalem Animal Healing will send patients accounts to collections for balances not paid after receipt of two statements unless you make payment arrangements with our billing office. We reserve the right to require payments for services to be made at or before time of service.

Outstanding balances: We may refuse to see patients with balances over $250, and who are not making regular payments on the balance. In the event that your account is placed for collection, a collection fee will be added to your account, along with any attorney fees and/or court costs that may be necessary for recovery of the outstanding balance. In the event of an NSF check, there will be a $30 NSF charge added to the balance due.

Surgery: The Surgery Coordinator will explain a cost estimate, which shows your financial responsibility. All elective procedures must be paid in full at time of arrival, all other procedures will be referred to our billing department.

I authorize Nehalem Animal Healing to keep my signature on file and to charge my credit card (held in our secure system) for charges associated with payment arrangements. Contact billing office to make payment arrangements.

Attestation Statement:
I have read, understand, and agree to the above Nehalem Animal Healing Payment Policy. I understand that charges not covered by Tillamook Animal Shelter or United Paws are my responsibility. I acknowledge that these policies do not obligate Nehalem Animal Healing to extend Credit.
